Had swine flu. I've been off work more than seven days, how do I get a back to work sick note?

Was off work for a week. I am now a bit stuck on the rules for going back to work. I know I need a sick note which the doctor will give me as I took tamiflu and feel better, but can a back dated sick note (covering the time I have been off) be accompanied with a fit to work statement/note in the same appointment??
